How Accurate Are Smartwatch Heart Rate and SpO2 Sensors?

Budget smartwatches measure heart rate and blood-oxygen (SpO2) from the wrist with a green (and sometimes red) LED and a light sensor. It's useful data — within limits. Knowing the limits stops you from either trusting a bad reading or dismissing a good feature.
What wrist sensors do well
- Resting heart rate trend over weeks. A gradual rise or fall is meaningful even if any single reading is a few beats off.
- Sleep heart rate — you're still, so the sensor gets a clean signal.
- Relative effort during steady exercise — walking vs jogging vs running shows clear, useful differences.
What they do poorly
- Heart rate during fast, jerky movement — HIIT, weightlifting, cycling on rough roads. The watch loses the pulse and either lags or shows your cadence instead of your heart rate. A chest strap is far better for this.
- Absolute SpO2 numbers. Consumer wrist SpO2 can be several percent off and is easily thrown by cold hands, a loose strap, movement, or tattoos. Treat a single reading as a rough check, not a diagnosis.
- Anything medical. These are wellness features. A worrying reading means see a doctor with a proper pulse oximeter, not panic at the watch.
How to get the best reading
- Strap snug, one finger-width above the wrist bone — not loose, not cutting off circulation.
- Still and warm. Cold fingers wreck SpO2. Sit for a minute first.
- Clean the sensor glass on the back of the watch; sweat residue scatters the light.
- For exercise HR, tighten the strap one notch before you start and expect a few seconds of lag.
